They can cause a series of sequelae, mainly mechanical limitation in the mouth opening, and therefore, there is an indication for surgical reduction when the displacement of bone fragments interferes with the oral or aesthetic function. Reductions can be open, followed by osteosynthesis with plates and screws, or even through bloodless procedures, less invasive and without the need for osteosynthesis. However, the principle of autonomy must prevail as long as all information is duly passed on to the patient. This study objectived to report a case of reduction by intraoral access without osteosynthesis in a patient with a zygomatic arch fracture. The procedure was performed under general anesthesia, after the patient was informed of the possible sequelae and complications if he rejected the proposed treatment. Through Keen’s intraoral access, the fractures were reduced without complications and the patient was instructed to take proper care. After 30 days after surgery, the patient was recovered, performing social and work activities without functional or aesthetic sequelae.
